The flu vaccine is being offered at your child’s school again this autumn/winter. We will be visiting your child’s school on Wednesday 21st and Thursday 22nd of October 2026
Please make sure you complete the form below prior to us attending your child’s school. If you are unsure when we are attending the school, please do not hesitate to ask the school or contact us.

Your consent will remain in place for the entire flu season. If your child misses a first visit, you do not need to complete a second form.
